If you use this site to request an appointment a member of our administrative staff will contact you by phone upon receipt of your request.

New Patient Forms

Please complete the new patient forms prior to your first visit. These forms are available through this site as a direct link if you wish to complete your registration forms on our secure site. Should you prefer, we have made a PDF version of these forms available for you to download and print. Once they have been completed please return to our office via US mail or fax. We are also happy to mail these forms to you at home or work. In order to best serve you we request that forms be completed at least 24 hours prior to your initial appointment.

Do you have records at a previous dentist?

To further ensure a seamless transition into our practice at Dental Health & Wellness Boston it is also important for you to request your prior dental office provide us a copy of any existing X-rays and dental records. In order to facilitate this a blank records release form is available as a PDF. This release can be printed and completed before sending to your previous dentist and has already been completed with our information for your convenience.

Insurance and Billing Information

At Dental Health & Wellness Boston we will work to make your dental goals possible. If you have insurance we can submit claims to your insurance company directly and apply all payments made to your account. Co-pays and deductibles are requested at the time of service and any balance remaining after insurance payments will be billed to you directly.

Since sedated patients can often undergo treatment for longer at one sitting, multiple procedures can be completed in fewer visits.
